Job Description
Clinical responsibilities: to include level of responsibility for: Lead the enhanced care team and build successful links between key professionals. Provide and assess Enhanced Care supervision to patients who have been assessed as needing additional support on the wards due to acute confusion, mental health conditions, challenging behaviour or physical health conditions which are compromising the safety of the patient and/or others. As a clinical leader provide support to the management team, participating in service and policy development. Act as an effective role model and resource/advisor to colleagues, in the delivery of nursing care. Work autonomously within the parameters of the role, directing, leading and supporting junior staff and wider care team to implement the delivery of quality nursing care. Build and support colleagues to build positive therapeutic relationships with patients needing Enhanced Care Supervision. Manage and oversee the deployment of the Enhanced Care Team in the delivery of enhanced observations to inpatient service users and those waiting in A&E. Provide practical support and guidance (in collaboration with elderly care and mental health colleagues) to clinical teams Manage patients with complex clinical and mental health conditions that pose a risk to maintaining a conducive and safe environment. Ensure that patients are provided with accurate information about care in an accessible format to patients and their relatives / carers Ensure the accurate monitoring and recording of enhanced care, identifying precipitating and protective factors for risk, individualised to the patient.
